Reminder to parents to complete all required health forms

Each year, the School asks parents/guardians to complete a number of health-related forms and submissions. Many of these forms/submissions are required by law, and must be provided in a timely fashion. If your family has not completed any of the required health forms/submissions listed below we ask that you do so immediately.

  • Concussion Resources & Acknowledgement Form
  • COVID-19 Vaccine Mandatory Disclosure Policy
  • Health Forms Package (includes required updated health information and consent forms for each student as well as optional sign-up for the influenza vaccine)
